Corporate policy:  Client accepts and agrees to full responsibility for the accuracy of the transcribed and completed document due to the level of difficulty, technical information or coursework to be transcribed.  All formatting and information required by the client shall be made in writing via fax or E-mail to Express Document Services, Inc.  No format instructions shall mean standard transcription formatting will apply.  Voice files obtained through illegal or unauthorized taping will not be transcribed under any circumstances. No warranties or guarantees are made by Express Document Service, Inc or its staff. All clients will sign as verification of completion of documents and therefore, acceptance of liability. No cash refunds will be made under any circumstances.

1. Accuracy Guarantee & Disclaimer
Express Document Service, Inc. Services, Inc. guarantees 98%+ accuracy for all documents transcribed by Express Document Service, Inc. Services provided that:  The dictation occurred via a land-line  or a digital recording device approved by Express Document Service, Inc. Services. The dictator spoke clearly and audibly, as well as spelled uncommon phrases and patient names.  The account has been with Express Document Service, Inc. Services for more than four weeks.   The dictator and/or dictator's designee accurately and completely uploaded and/or transferred dictated voice files.  Dictator voice files have not been re-recorded at any time.

2. Turn-Around Guarantee & Disclaimer
Turn-Around refers to the time interval between the time Express Document Service, Inc. Services receives a dictated voice file from the customer and the transcribed document is returned to the customer via our web-based application/server site. Express Document Service, Inc. Services guarantees that 96% or more documents will be within the guaranteed turn-around time (typically 48 hours, but may vary on pricing package and customer requirements) provided that: No network or related breakdowns occurred which are outside Express Document Service, Inc. services control (including but not limited to Internet network failures, power failures, or customer's ISP service interruptions).  The dictator spoke clearly and audibly as well as spelled uncommon phrases and patient names.  The account has been with Express Document Service, Inc. Services for more than four weeks.  The customer has informed Express Document Service, Inc. Services at least two weeks ahead of time of any increases in transcription volume. The customer is not experiencing higher than normal transcription volumes. The customer provides Express Document Service, Inc. Services with a continuous, equal level of quantities in dictation. Please note that turn-around times maybe extended by up to 48 hours during national holidays.


3. Minimum Requirements For  accessing Express Document Service, Inc. Server:  Computer with Windows (R)2000 or higher. Internet Explorer 5.x or higher. Internet access, high speed (such as DSL, cable or T1) recommended.   Hand held devices are purchased by client unless otherwise agreed upon. These devices vary in cost from $250 to $350. Express Document Service, Inc. Services does not  maintain or support such devices. Additional software may be necessary which will be provided to customer by Express Document Service, Inc. Services for no additional cost.

4. Other:   The server based application is free to dictators who transcribe at least 1,000 lines with Express Document Service, Inc. Services per month.  Customer will have access to the Web application until the end of the billing period in which the account was canceled
